Kingdom Hearts HD II.8 Final Chapter Prologue collects the previously released Kingdom Hearts: Dream Drop Distance (Nintendo 3DS) and Kingdom Hearts Birth by Sleep Final Mix (Sony PSP) and adds all new content in the way of  Kingdom Hearts χ Back Cover,  which follows the Foretellers in HD cinematics and Kingdom Hearts 0.2 Birth by Sleep–A Fragmentary Passage, which takes place after the events of Birth by Sleep and is told through the eyes of Aqua. With these two new pieces of content, Back Cover and A Fragmentary Passage effectively become the ninth and tenth installments in the Kingdom Hearts series.

The PlayStation 4 exclusive will be released worldwide in December 2016.

As for Kingdom Heart 3, it's anyone's guess as to when the next numbered entry in the franchise will be released.  Kingdom Hearts 2 was released in 2005 and concepts for KH3 were shown at E3 2006 but after ten years, it still seems as if the game is a long ways off.
